Well, it depends of your ISP (Internet Service Provider) i guess, and also of the distance between you and the person that's trying to join you (in example: a connection between USA and Europe isn't really possible usually). It might be also because of the Quake2 engine you're using.
Tl;dr:
1) If the person that tries to connect you isn't near you - try using Hamachi.
2) If the person is near you , it's because you cannot host a game
- well, Hamachi might be usefull too here.
3) If you're using another Quake 2 engine (like EGL), try putting this command into your launcher at the end of the last line:

+set net_compatibility 1
Note: don't forget to put the space too! (before the "set net_compatibility 1" command)
Note 2: to open your launcher, right click on the thingy you use to open coopordie:

and then click on Edit.
